Term:                                 Maternity replacement (preferred start date December 1, 2019 14 month contract)

Job Summary:
The Clinical Coordinator of the Hospice Residential Home will support and work towards achieving the strategic directions set out by the Board of Directors of the Hospice of Windsor and Essex County Inc. The Clinical Coordinator of a Hospice Residential Home will be assigned one primary base location, Windsor or Leamington (Erie Shores), and will also provide coverage to support the needs of either campus as necessary for after hours, back-up or urgent issues. Performance is evaluated on an annual basis.

Principal Duties and Responsibilities:
1. Communicate, implement and ensure that Hospice’s responsive and holistic culture is maintained and evident within the residential home.
2. Develop and nurture professional and therapeutic relationships with community partners.
3. Communicate and implement any of the Hospice’s support services if appropriate for families and patients admitted to the residential home.
4. Discuss with families the various opportunities for memorial donations in order to strengthen Hospice’s financial position
5. Daily management of the residential home, monitoring care & service to ensure the provision of safe services and quality patient care.
6. Apply knowledge in palliative clinical practice in the mentorship in all clinical personnel related to specific resident care.
7. Provides leadership related to resolution of occurrences and conflict management for all team members – contracted service providers, Hospice personnel, and Hospice volunteers, in promotion of ongoing high quality resident care.
8. Fosters an interdisciplinary team environment in the promotion of best quality resident care.
9. Acts as the Hospice liaison with contracted Service Providers (ex. Bayshore), Erie St Clair Local Health Integration Network- Home and Community Care (LHIN), local hospitals, and other direct Service Providers and agencies as necessary.
10. Collaborate with the Hospice Palliative Physician team in order to: provide best quality palliative care to residents.
11. Support Medical Residents, Fellows, Nursing Students, and Interns, in the knowledge development and promotion of palliative care practice in the residential home.
12. Complete annual performance evaluations for assigned personnel.
13. The Clinical Coordinators for both campuses will provide cross coverage and will rotate between the two campuses as needed.
14. The Clinical Coordinator assigned to the Leamington (Erie Shores) location will provide back-up coverage to support the Palliative Liaison Program as required at Erie Shores Health Care.
15. The Clinical Coordinators for both Residential Home Campuses will work together to ensure that the residents of Windsor and Essex County are waitlisted and admitted to either campus in a timely fashion.
16. Oversee the HRH waitlist; provide monthly, quarterly and yearly statistical reports and ministry reports as required or requested.
17. Assess for education needs and assist to provide education to staff and volunteers in collaboration with Centre of Excellence.
18. Ensure a safe environment for clients/family, staff and volunteers.

1) Accountable to the Director of Patient and Family Clinical Services.
2) Performs in adherence to the Hospice Mission, Vision and Values.
3) Adheres to the Hospice Policies and expectations related to professional behavior.
4) Maintains clinical competence through continuing education and reflective practice protocols as established by the College of Nurses of Ontario.
5) Submits confirmation of annual registration with the College of Nurses of Ontario to the Hospice Human Resources Department prior to December 31st of each calendar year.
6) Conducts practice in accordance with the professional standards outlined by the College of Nurses Ontario.

1. Registration with the College of Nurses of Ontario as a Registered Nurse.
2. Degree in Nursing preferred.
3. Management or Leadership training and/or direct experience managing clinical personnel preferred.
4. At least five (5) years’ experience in oncology and/or community palliative care preferred.
5. Completed the “Fundamentals of Palliative Care”, or comparable Hospice Palliative Care Training. Comparable experience in providing palliative care consistently for a minimum of five (5) years may be considered.
6. Completion of the “Comprehensive Advanced Palliative Care Education” (CAPCE) course and the “Learning Essential Approaches to Palliative and End of Life Care” (LEAP) course in advance of employment start is preferred. However, this may be completed within one (1) to two (2) years following the start of employment.
7. Ability to speak fluently in the French language (bilingual English/French) is an asset.
8. Proficient in computer use including Microsoft Office Word, Outlook, and Excel.
9. Excellent oral and written communications skills.
10. Good organizational and prioritizing skills.
11. Ability to provide active conflict management and mentorship
